Requesting Your Health Care Records

You are entitled to request a copy of your mental health records.

To obtain your records, please email directly to the practice. You can submit requests through a phone call or the secure client portal. Records will be supplied in compliance with Texas law and relevant privacy regulations to supply clients with their records within 15 business days of receiving a written request.
